Please use this form to report an incident as soon as it occurs, no matter how big or small. This includes accidents, near-misses, equipment damage and any event that has resulted in an injury. Complete the form immediately after the incident or arrange for someone to do it on your behalf. The incident report will ensure that all critical information in relation to the incident and person(s) involved are well recorded in a timely manner. When completed, press submit and this will then be logged by the Club's Incident Reporting Manager.
