In the event of an accident, the following procedure should be followed:
- Make contact with parents/guardians
- Contact emergency services/GP if required
- Record in detail all facts surrounding the accident, including details of witnesses etc.
- Fill in our online form OR complete two copies of the hardcopy form (see links below)
If a hardcopy form is completed, please retain one copy of the form and include this in the incident book / folder. Please scan and email the other copy to firstname.lastname@example.org
